Percentage of Faults with Root Cause Identified measures the proportion of faults for which a root cause is determined during or after resolution. This KPI reflects the effectiveness of root cause analysis (RCA) in preventing recurrence of issues. Identifying root causes in at least 90% of resolved faults helps ensure continuous improvement and reduces the likelihood of repeat faults in the future.
